TEMPORARY STAFFING AGREEMENT

All persons assigned to your property through our staffing service for temporary assignments will be paid by MLCHS'S PEO which will assume all associated payroll taxes, benefits, and deductions.

The client's billed rate is equal to the employee's hourly rate x 1.6:

TEMP-TO-PERM ROLLOVER AGREEMENT

Your club may hire that person for permanent employment after 560 hours has elapsed subject to certain terms and conditions. Upon completion of 560 hours, the employee may rollover to a permanent position if the client desires at a fee of 8% of the contract agreement.

Your club is not obligated to hire an employee upon the completion of the 560 hours. If for any reason the person does not complete the 560 hour period, a new person will be provided to complete the balance of the 560 hours.

Should your club desire to permanently hire an employee before the 560 hours has elapsed, you may do so for the following cost:

560 hrs. less hours worked x bill rate x 25%
